Client files are held in a secure filing cabernet which is accessible only by your psychologist. Information on client files includes information such as name, address, phone numbers, as well as other information which is relevant to the psychological service being provided.
The information is gathered as part of the assessment, diagnosis and treatment the client and is seen only by the psychologist. The information is retained in order to document what happens during sessions, and enables the psychologist to supply relevant and informed psychological service.
All personal information gathered by the psychologist during the provision of psychological service will remain confidential and secure except where:

Failure to disclose the information would place you or another person at serious or imminent risk
Your approval has been obtained to release the information to another person or agency.
